Responsibilities
JOB SUMMARY
This job is responsible for collecting current and accurate information from patients/families with respect to home medications prescribed for, and/or taken by, the patient and for entering collected data into the electronic medical record in accordance with established procedures. Incumbents produce an admission medication reconciliation sheet in the electronic medical record to be presented to a pharmacist for review and approval, and subsequently made available for physician review and approval if appropriate.
Work is accomplished by exercising effective customer relations skills in interviewing patients/family members directly, frequently in stressful situations. Incumbents collaborate with the inpatient pharmacy department (pharmacists, technicians, and assistants) and nursing staff to relay information to facilitate patient care activities, as well as physician offices and retail pharmacies to collect/validate patient medication information to ensure complete accuracy. Responsibilities include improving the mediation reconciliation process (accuracy and speed of completion) to facilitate compliance with Joint Commission and Department of Health regulations regarding the collection and reconciliation of home medication information.
ESSENTIAL JOB FUNCTIONS:
- Interviews/gathers/collects information regarding all current home medications from patients or caregivers, primarily in the Emergency Department; asks pertinent questions relative to dosage, to obtain clarification of information and to facilitate the timely/accurate collection of data.
- Enters information collected from interview activities into the electronic medical record in accordance with established procedures for purposes of generating admission medication reconciliation documentation.
- Produces documentation for review by pharmacist and subsequent physician review.
- Provides assistance to other healthcare providers in the ED and other locations to facilitate the provision of medications on an as needed basis, working with the inpatient pharmacy staff as necessary.
- Cultural Sensitivity and Competence: Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Population Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.
- Performs related duties as required.
Qualifications
Education/Work Experience Requirements
Graduation from an accredited Pharmacy Technician program, AND one of the following: 1) one year of pharmacy technician work experience in either an inpatient or ambulatory setting, OR 2) one year of work experience in a customer service-related field, working directly with customersthat demonstrates attainment of the requisite job knowledge skills/abilities.
Licensure/Certification
- Licensed as a Pharmacy Technician by the Washington State Board of Pharmacy.
- Current healthcare provider BLS Certification.
