Responsibilities

At CommonSpirit Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As the Director, Population Health Services, you'll be responsible for driving the success and effective use of CommonSpirit Health at Home’s healthcare delivery capabilities,  focusing on cost of care and quality of care across our homecare and hospice settings.  Other responsibilities include:

  • Develops close working relationships and trust within the Organization’s culture and structure, to meet or exceed the home health strategy, care management at all levels, and population health goals set forth by the organization.
  • Collaborates on opportunities for improved performance. Works with key internal stakeholders, including all organization service lines including but not limited to, finance, revenue cycle, legal, population health, care management, physician enterprise, strategy, corporate responsibility and population health team members, and other key clinical and business leaders per market.
  • At the Homecare and Hospice service line level, develops a consistent Utilization Management program to meet key operational, clinical and utilization metrics.
  • Serves as the subject matter expert for utilization management across the care continuum, including interpretation and guidance for new and emerging innovations, rules and regulations from federal, state and commercial payers.
  • Assists Homecare and Hospice staff with tools to assist, better understand, manage and engage populations to improve overall population health of the communities they serve and to deliver higher quality care at a lower cost.
  • Coordinates data sharing between the clinical, operations and business development teams, legal, corporate responsibility, information technology, finance, revenue cycle, and other population health management stakeholders. Monitors performance in these programs and provides updates to leadership. Develop creative reimbursement options with hospital partners and payers as the organization moves more toward value based reimbursement, while working with compliance and legal.
  • Serves as advisor to operations across the continuum of care for all new CMS innovation payment programs. This includes participation in program development, facility network development and relations, and physician network development and relations; patient tracking across the continuum of care; and decisions regarding homecare and hospice alternatives to acute care which consider quality and financial factors.
  • All other duties as assigned.

Qualifications
  • Bachelor of Arts or Science Degree required; Bachelor of Science in Nursing preferred. 
  • Minimum two years’ experience as hospital Care Transition Nurse or Client Relations Executive in home health or managing health care sales staff required.
  • One year of population health management/value based purchasing experience preferred.
  • Ability to travel is for extended periods required with this position.
  • Excellent understanding of Medicare regulations for home care.
