Responsibilities

The Pharmacist is responsible for providing pharmaceutical care within a clinical setting, ensuring the safe and effective use of medications for hospitalized patients. This role involves reviewing and dispensing prescriptions, monitoring drug interactions, advising healthcare providers on appropriate drug therapy, and counseling patients on medication use. The pharmacist also participates in medication therapy management, works closely with medical teams to optimize treatment plans, and ensures compliance with hospital policies and regulatory standards. Additionally, the pharmacist educates patients and staff on the proper use of medications, tracks inventory, and manages the preparation and dispensing of both sterile and non-sterile medications. Strong attention to detail, clinical knowledge, and communication skills are essential for ensuring the best possible patient outcomes.

  • Ensure the appropriate handling, processing and dispensing of all designated high risk medications in accordance with hospital policy.
  • Reviews and verifies or enters medication orders into the computer system in an accurate and timely manner.
  • Assures entries to the patient medication profile present no significant problems in regard to drug interactions, drug-allergy interactions, incompatibilities, therapeutic duplications or patient safety, and intervenes as appropriate.
  • Reviews medication orders and evaluates their appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, laboratory values, pregnancy and lactation status and other pertinent patient information.
  • Ensures the timely clarification of medication orders. Contacts the prescriber with suggestions for improvement and resolves problems with the medication orders.
  • Prepares, verifies and dispenses medications in a timely and accurate manner, utilizing available safety features (eg, barcoding) as outlined in our facility policy.
  • Prepares, verifies and dispenses medications for parenteral use (including chemotherapy and parenteral nutrition) in a timely and accurate manner, utilizing appropriate aseptic technique as outlined in USP 797 and our facility policy.
  • Demonstrates knowledge in the relationship of age and the proper selection of drugs, dosages, routes of administration and drug pharmacokinetics for patients in the following age-specific categories: Neonate/Infant, Pediatric, Adolescent, Adult, and Geriatric.

Qualifications

Minimum: 

  • Bachelors Degree in Pharmacy or Pharm D
  • Valid Arizona Pharmacist License 

Preferred: 

  • (1) year of experience as a Pharmacist at a hospital
