Social Worker - Care Coordination
St. John’s Hospital Camarillo
St. John’s Hospital Camarillois seeking a compassionate and experienced Social Worker to join our dynamic Care Coordination Department. This role requires a proven track record of success in care coordination and case management within a healthcare setting. The ideal candidate will be a strong advocate for our patients, possess excellent communication skills, and thrive in a fast-paced environment.
Responsibilities
- Conduct comprehensive psychosocial assessments of patients to identify needs and develop individualized care plans.
- Collaborate with the interdisciplinary care team (physicians, nurses, therapists, etc.) to coordinate and implement care plans.
- Advocate for patients' needs and rights, ensuring access to appropriate resources and services.
- Provide counseling and support to patients and their families, addressing emotional, social, and practical challenges.
- Facilitate communication and collaboration among healthcare providers, family members, and community resources.
- Monitor patient progress and make necessary adjustments to care plans as needed. Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate in case conferences, team meetings, and continuing education opportunities.
- Assist with discharge planning and post-discharge follow-up.
Qualifications
- At least one year of clinical and/or medical Social Work experience is required.
- Master knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse, neglect, or violence; community or financial resources for underserved or vulnerable populations.
SSDI, SDI, and other governmental/county requirements/programs. Thorough comprehension of OBRA and Title 22 is required in California. - Excellent customer service and presentation skills are a must.
- Strong interpersonal and written communication skills are essential Demonstrated ability to apply analytical and problem-solving skills Demonstrated ability to manage multiple tasks or projects effectively
- Ability to work independently as needed with a high degree of detail orientation.
- Ability to work efficiently in a fast-paced environment with changing priorities.
- Degree from a School of Social Work accredited by the Council of Social Work Education
This position is represented by SEIU-UHW and is covered by the terms and conditions of the collective bargaining agreement.
