CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services.  CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. CHI St. Alexius Health offers a comprehensive line of inpatient and outpatient medical services, including: a Level II Trauma Center, primary and specialty physician clinics, home health and hospice services, durable medical equipment services, a fitness and human performance center and ancillary services throughout western and central North Dakota.

Responsibilities

STATEMENT OF PURPOSE: Under the guidance of the Director of Outpatient Pharmacy Services, the Clinical Pharmacist is responsible for assessing and monitoring the pharmaceutical needs and progress regarding drug therapy to the Infusion Therapy and Respiratory Care Medication Programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Attains completion of department goals/objectives as identified at last performance appraisal 
  • Successfully completed the defined job specific competencies.  (List the job specific competencies validated during the year in the comment section.) 
  • Oversees and directs the shipping of all RT and Infusion Therapy Medications
  • Assures all prescriptions are up-to-date and renewed yearly.
  • Original prescriptions are on file with billing and copies are kept with med program coordinator for quick reference.
  • Verifies patient information to assure correct patient information available for shipping medications.
  • Arranges convenient delivery method.
  • Prepares/orders medication from St. Alexius Pharmacy for next day’s deliveries by end of day.
  • Prepares and or validates all medication dispensing to assure medication is dispensed and addressed correctly
  • Organizes, manages, and pharmacy services for recipients of RT Medication Program and Infusion Therapy patients 
  • Prepares medication histories for the patient’s permanent medical record or other databases (e.g. medication profiles), or both.
  • Provides oral and written consultations with other healthcare professionals regarding drug therapy selection and management.
  • Monitors drug therapy and communicates relevant findings and recommendations to other healthcare professionals who are also responsible for the patient’s care.
  • Evaluates and develops patient-specific drug therapy management plans and therapy endpoints.
  • Monitors, detects, documents, reports, and manages adverse drug reactions.
  • Educates and counsels patients and family members regarding drug therapy and drug-related disease prevention.
  • Maintains access and communication with discharge planner and social workers to provide for smooth discharge.
  • Coordinates and provides educational information to practitioners to maintain an ongoing understanding and appreciation of Respiratory Therapy and Home Infusion services

  • Patient Assessment and Evaluations 
  • Communicates with physician on care the patient is receiving, making recommendations for improvements in therapy, and/or lifestyle changes, when appropriate.
  • Evaluates patients and develop plans and revisions to meet the patient’s needs.
  • Identifies age specific educational needs for patient/significant other with documented evidence of patient/significant other response to education notes.

Qualifications

Education: Graduate of a College of Pharmacy with a Baccalaureate Degree or PharmD 

Licensure: Licensed and currently registered as a Pharmacist in the State of North Dakota in good standing. Current CPR certification. 


Experience: One (1) year hospital residency or fellowship in Clinical Pharmacy practice or two (2) years experience as hospital pharmacist or related field.
