Responsibilities

The Community Health Navigator at St. Bernardine Medical Center plays a vital role in patient/client advocacy, care coordination, and community resource navigation. This position supports uninsured individuals, inpatients, and community members within vulnerable populations by guiding them through the healthcare system and ensuring access to essential medical, social services, and community outreach programs. The Community Health Navigator provides follow-up support and works to connect individuals with Federally Qualified Health Centers (FQHCs), low-cost and free clinics, and other critical resources to promote ongoing healthcare access.

This role focuses on building relationships, solving problems, and educating patients/clients on self-navigation within the healthcare system and community outreach programs. By providing culturally and linguistically appropriate health education support, the Community Health Navigator helps manage their health and improve their overall quality of life. While this position does not provide clinical care or replace the expertise of medical professionals, it is essential in reducing healthcare disparities, supporting patient empowerment, and advancing St. Bernardine Medical Center’s mission, core values, and strategic initiatives.


Qualifications
  • Associate Degree, preferable in a healthcare related field preferred
  • Experience in a healthcare setting working with patients, healthcare services, medical groups, and payers is required.
  • Ability to use Google Office Suite, Microsoft Office Suite (specifically Word, Excel, Outlook and Access).
  • Demonstrate ability to navigate the internet and Windows-based software applications.
  • Able to provide culturally appropriate service.
  • Ability to build effective relationships with patients staff community agencies and community members.
  • Knowledge of healthcare payors, medical groups and services.
  • Understanding of medical terminology
  • Good written and verbal communication skills. Bilingual Spanish speaker with ability to read and write Spanish preferred/\.
  • Valid California Drivers License in good standing required.
