Overview

AmeriMed provides home infusion therapy services to help patients safely transition care to their home, alleviating long and unnecessary stays in the hospital. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy

As a PRN Care Transition Nurse, you will play a vital role in seamless transitions from hospital to home, ensuring that patients receive the care they need efficiently and effectively.
**Key Responsibilities:**
- Collaborate with discharge coordinators and healthcare teams to ensure high-quality home care services are coordinated for patients.
- Assess patient eligibility for our infusion services and provide expert guidance to healthcare providers for smooth transitions.
- Maintain effective communication with CommonSpirit Health at Home staff to ensure a comprehensive approach to patient care.
- Build strong professional relationships with discharge planners, physicians, and other hospital personnel to foster a supportive network.
- Educate patients and their caregivers about infusion services, insurance benefits, and procedural requirements, promoting informed choice and understanding.
- Order necessary equipment for home infusions in coordination with patient needs and treatment plans.
- Participate in onboarding and training new staff on home infusion processes, enhancing team knowledge and efficiency.
- Provide general infusion education as requested by referral sources, empowering patients and caregivers with knowledge.
- Ensure continuity of care by collaborating with the operations team and attending regular meetings.
- Work across diverse clinical areas, including neonatal, pediatric, adolescent, adult, and geriatric populations.

Qualifications

  • Must be a graduate of an accredited school of nursing.
  • Experience as a Care Transition Nurse or similar role, sales or marketing.
  • Hospital and/or homecare nursing experience.
  • Current RN license to practice in the state serviced.
  • Obtain and maintain current CPR certification; able to perform CPR.
  • Compliant with appropriate state certification/registration and organizational requirements as necessary.
  • Two years of appropriate nursing experience.
  • Knowledge of home care regulations required.
  • Strong assessment, critical thinking, judgment, creativity and decision making skills.
  • Excellent communication skills; both verbal and written.
