**This posting is for the 2025-2026 ASHP Accredited Postgraduate Year Two (PGY-2) Pharmacy Residency Program**

Baylor St. Luke’s Medical Center is an 881-bed quaternary care academic medical center that is a joint venture between Baylor College of Medicine and CHI St. Luke’s Health. Responsibilities

Provides integrated, contemporary pharmacy services to patients, to include the interpretation of medication orders, dispensing of pharmaceuticals, clinical monitoring and evaluation of drug therapy, and the provision of drug information to patients and healthcare providers.  

  • Reviews, interprets, and evaluates all medication orders for patients in all age groups.
  • Maintains patient medication profiles and medication administration records in an automated pharmacy information system.
  • Consults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of pharmaceuticals to help resolve drug therapy related issues.
  • Routinely monitors the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.
  • Participates actively in cardiopulmonary emergencies.  Must be “code validated”  
  • Conducts assigned target drug monitoring programs, quality improvement initiatives and concurrent drug use evaluation activities as required.
  • Performs and documents Pharmacist Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy as well as follow up.
  • Obtains medication histories on selected patients and communicates pertinent information to the appropriate healthcare provider.
  • Communicates with physicians, nurses, and other healthcare providers as necessary.
  • Completes at least one major and one minor residency project focusing on appropriate drug therapy, improving patient outcomes, or performance improvement activities.
  • Present at least one major seminar to the health professional staff.
  • Review and write up at least one drug information question.
  • Participates in special projects or performs other duties as assigned.

Qualifications

REQUIREMENTS:

  • Registered Pharmacist with the Texas State Board of Pharmacy in good standing-(RPH)
  • Aseptic Technique – Complete an ACPE approved 20 hour sterile products course within 90 days of hire
  • Completion of ASHP Post Graduate Residency (PGY-1) program
