CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services.  CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. CHI St. Alexius Health offers a comprehensive line of inpatient and outpatient medical services, including: a Level II Trauma Center, primary and specialty physician clinics, home health and hospice services, durable medical equipment services, a fitness and human performance center and ancillary services throughout western and central North Dakota.

Responsibilities

STATEMENT OF PURPOSE: Collaborate in all phases of pharmaceutical services that includes drug distribution and clinical services, respecting all persons for their own personal worth and dignity while maintaining cost effectiveness

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Pharmacist Responsibilities 
  • Monitor drug therapy and communicates findings and recommendations to appropriate professionals
  • Collect PharmDoc intervention data and submit on an ongoing basis
  • Actively participate in renal dosing protocol
  • Accurately assess medication orders for appropriateness (dose, frequency, indication, drug-drug interactions)
  • Actively work to help direct nursing staff as to how to resolve problems/errors
  • Actively address & resolve items in Pharmacist work queue in Pharmacy Order Entry System
  • Actively participate in registering & following Clozapine patients in IVAX
  • Able to be scheduled to cover an area in addition to your primary responsibility (IS, OR, CP, IP, Clinical)
  • Able to be scheduled for a shift in a pharmacy order entry area.
  • Actively participate in pharmacokinetic monitoring
  • Actively participate in monitoring/calculating adult TPN
  • Actively participate in monitoring/calculating neonatal/pediatric TPN
  • Actively participate in anticoagulant management services
  • Actively participate in pain management
  • Actively participate in monitoring microbiology cultures
  • Actively participate in IV to PO switch program
  • Prepare/check intravenous admixtures
  • Drug Information/ADRs/Med Errors
  • Educate/counsel patients and customers about their drug regimen(s) and/or disease prevention
  • Provide  timely drug information
  • Document  drug information questions on drug information database as defined per policy 
  • Actively participate in ADR reporting process
  • Actively participate in medication error reporting process
  • Make recommendations for improving safety in the medication use system
  • Formulary Management
  • Review a class of medications
  • Complete at least one DUE project per year
  • Present one or more items to Pharmacy & Therapeutics Committee per year
  • Promote Formulary Management and support Formulary Management decisions
  • Develops formulary drug monograph for formulary addition request and makes recommendation for P &T
  • Policies and Standardized orders
  • Participate  in writing, revising, or reviewing two or more departmental/house-wide policies and procedures you are responsible for.
  • Educate appropriate staff about new or revised policy and procedures
  • Follow current policy and procedures
  • Participate in review and revision of physician standardized order sets
  • Maintains Professionalism, Competencies and Leadership Standards by 
  • Present and/or helps develop an in-service for the department or organization
  • Serve as a committee/group/Performance Improvement Team (PIT) chairperson or co-chairperson
  • Serve as a student or resident preceptor
  • Participate  in department education by writing departmental newsletter article as assigned
  • Actively support and/or participate in clinical research
  • Staff development
  • Successfully complete the defined job specific competencies, including competency for age specific, as assigned. 

Qualifications

Education: Graduate of a College of Pharmacy with a minimum of a Baccalaureate Degree in Pharmacy.

Licensure: Licensed as a pharmacist in the State of North Dakota in good standing.
