Bruceville Terrace has been providing quality compassionate care since 1989. Licensed as the 171-bed sub-acute skilled nursing facility of Methodist Hospital of Sacramento Bruceville Terrace offers care for the elderly as well as those requiring extended recoveries. 

Bruceville Terrace provides skilled nursing rehabilitative and complex medical services to our patients and residents using an interdisciplinary team approach. Over half of the facility is dedicated to skilled nursing/convalescent care while the remaining beds are committed to meet the medical and rehabilitative needs of the short-term patient after a stay at the acute care hospital. Responsibilities

Job Summary:

The incumbent provides skilled, technical nursing care or service to patients and residents according to a plan of care established by a Registered Nurse and in accordance with current federal, state and local standards, guidelines and regulations that govern the facility and as may be required by nursing
management. Responsible for accurately and effectively completing all other duties as assigned. 100% of the incumbent's time will be spent at Bruceville Terrace.

Job Duties:

  • Accurately collects admission data and records information on Admission/Assessment/History Recordwithin 2 hours of arrival on unit.
  • Delegates data collection appropriately.
  • Observes and reports to the Registered Nurse, patient's responses to therapeutic interventions hospitalization and diagnosis, including physical and psychological response.
  • Collaborates with the interdisciplinary team in meeting identified needs by appropriate referrals.
  • Collaborates with the Registered Nurse regarding discharge planning.
  • Demonstrates individual competency in selected psychomotor skills and independently performs technical procedures such as demonstrated ability to carry out procedures, assist physicians with procedures, safely use equipment, and proficiency on HIS.
  • Completes annual competency testing within the time frames specified.
  • Demonstrates ability to carry out medical orders as directed by the nursing management team by considering appropriateness of orders in relationship to current therapy and integration with nursing standards.
  • Demonstrates ability to administer medications appropriately and demonstrates competence in performing duties of lower-level patient care team members as required to meet needs of patient/unit.
  • Demonstrates effective response to unit/patient crisis situations by initiating appropriate procedures, protocols within the scope of LVN licensure and alerts Registered Nurse appropriately. Anticipates patient/unit needs during emergency/code situations without requiring direction.
  • Demonstrates knowledge of disease processes and the rationale supporting patient care tasks by collaborating with the supervisor or designee on changes in patient condition in a consistent and timely manner.
  • Demonstrates appropriate organization, priority setting, and problem solving skills to meet assigned duties by prioritizing work load to avoid incidental overtime not to exceed 2% to 3% of hours worked.
  • Implements a plan of care as directed by a Registered Nurse by providing input into the development of the patient's plan of care. Collaborates with the Registered Nurse for implementation and modifications in the plan of care, protocols and pathways.
  • Assists the patient/family in learning appropriate self-care techniques as directed by the Registered Nurse, Teaching Protocols, and established Interdisciplinary Plans of Care/Clinical Pathways, recognizing developmental, age, physical and cultural needs.
  • Performs timely, concise, and accurate documentation by writing legibly at all times to include signature, title date and time and documenting all care given to the patient on appropriate documentation form according to established guidelines. Utilizes the electronic medical record to document care delivery and response as indicated.
  • Assures that resident's right to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints daily by reporting immediately any suspected or observed incidents of resident abuse per our facility policy and procedures.

Qualifications

Minimum Requirements:

  • 1 year of SNF/LTC care of the aged acutely ill chronically ill and patients in need of restorative care.
  • Knowledge in the principles of nursing care within the LVN scope of practice.
  • Ability to communicate proficiently both orally and in writing in the English language.
  • Current Licensed Vocational Nurse.
  • BLS certified by the American Heart Association.
