Responsibilities

The Patient Support Services (PSS) Clinical Social Worker provides individual and group therapy sessions to individuals living with HIV in a multidisciplinary outpatient medical clinic. This role involves providing behavioral health interventions in various evidence-based modalities, conducting assessments, developing intervention plans, and coordinating care to enhance patients' overall well-being and treatment adherence.

  • Conduct thorough biopsychosocial assessments to identify client needs and develop comprehensive, multidisciplinary PSS Intervention Plans in collaboration with patients and the healthcare team.
  • Provide individual and group therapy sessions, as well as facilitate support groups, addressing issues such as new diagnosis, disclosure, treatment adherence, mental health concerns, and grief and loss.
  • Conduct thorough biopsychosocial assessments to identify client needs and develop comprehensive, multidisciplinary PSS Intervention Plans in collaboration with patients and the healthcare team.
  • Develop and maintain a supportive and therapeutic relationship with clients.
  • Actively participate in a multidisciplinary care team, communicating effectively with medical providers, case managers, and other healthcare professionals to ensure comprehensive, patient-centered care.
  • Respond to immediate mental health crises during clinic visits, providing triage, de-escalation, and appropriate referrals to ensure patient safety and continuity of care.
  • Identify and connect patients with appropriate community resources for mental health services, substance use treatment, housing assistance, transportation, and other social support needs.
  • Document client progress and treatment plans in a timely and accurate manner.

Qualifications
  • Experience working with clients who have a history of trauma, substance abuse or mood disorders.
  • Master’s Degree in social work, counseling, psychology or related field.
  • Eligible to be licensed as a State Mental Health Professional.

Any of the following are acceptable:

  • ASW - Associate Social Worker
  • MFT - Marriage Family Therapist
  • LCSW - Licensed Clinical Social Worker
  • LPCC - Licensed Professional Clinical Counselor
