Responsibilities

Job Summary:

Provides psycho-social assessments, counseling and referral to community services and resources. Educates patients, families, and Woodland Healthcare staff. The Social Work department is a self-managed work team. The social workers are on call weekends and holidays. 

  • Provide psycho-social assessments for inpatient and outpatient units of Woodland Healthcare.
  • Counsels patient and families for psychosocial-related issues.
  • Refers patient and families to community services and resources.
  • Acts as a resource for education and community services for patients, families, and Woodland Healthcare staff.
  • Participates in Quality Control and Process Improvement activities.
  • Participates in interdisciplinary rounds and special conferences. Services on Woodland Healthcare and community committees.
  • Supervises MSW interns.

Qualifications

Experience:

  • A minimum of one (1) year of acute care hospital experience of a combination of hospital and other clinical experience which provides for the following knowledge, skills and abilities.
  • Knowledge of: The principles and practices of clinical social work in the healthcare setting, Individual and group consultation process, Principles of grief and loss counseling, Crisis intervention techniques, Patients rights, ethical considerations and cultural differences in dealing with the hospitalized patient, Community health, welfare, and social agencies, Chemical dependency and mental disorders.

Education: 

  • Masters degree in Social Work

Training: 

  • Ability to: Counsel patients and families for emotional, social and physical issues by assisting them in coping with feelings and fears during hospitalization or outpatient treatment.
  • Demonstrate knowledge and skills relevant to the principles of growth and development over the life-span of the patient. Ability to interpret knowledge needed to identify each patient's requirements relative to is or her age specific needs.
  • Perform psychosocial assessments and formulate appropriate plans,
  • Assess and report incidences of suspected child abuse, dependent adult/elder abuse and domestic violence, Develop and maintain an empathetic environment with patients and families, Communicate effectively with the hospitalized patient and family members.
  • Communicate psychosocial needs of the patient and family to the interdisciplinary team, Develop and maintain a professional working relationship with Woodland Healthcare and medical staff members.
  • Develop and maintain a professional relationship with community health, welfare and social agencies, Maintain patients medical records, Maintain confidentiality. Actively participate in the Self-Managed Work Team process.
  • Participate in hospital committees.
  • Plan and organize work efficiently.
  • Function independently and seek consultation from colleagues when appropriate.
  • Maintain professional development by participating in continuing education.
  • Provide education to staff.
  • Provide supervision to MS students, as necessary.
