Responsibilities

Under indirect supervision the incumbent provides direction and management for all operations of the Adult Day Care Department including but not limited to Adult Day Health Care, Adult Day Care and care management programming. The position reports to the hospital president. Reporting directly to the program director is RN Supervisor, Social Work Supervisor, Program Supervisor and Clinical Coordinator.

Job Duties: 

  • Ensure that all requirements listed in Title 22, California Administrative Code, Sections 54323, 54405, 78313 and 78417 are met including:
    A. All appropriate and needed services are provided to participants
    B. Timely initial and ongoing assessment of participants
    C. Person-centered plans of care are developed and implemented for all participants and reviewed quarterly by the interdisciplinary team.
    D. Daily documentation of all treatments and services
    E. Ongoing liaison with families and physicians is provided
    F. Emergency care including transportation is provided when needed
    H. All State mandated reports are completed
    I. Utilization Review process is maintained
  • Responsible for departmental staffing with attention to hiring qualified and well-trained staff, department staffing budget, Title 22 regulatory ratios and productivity targets. Monitors staffing levels and scheduling at all times to ensure maximum flexibility and cost-effective coverage.
  • Develops a strong team philosophy within the department that encourages collaboration, interdepartmental communication and sensitivities to cultural and other differences.
  • Directs the development, implementation and monitoring of strategic goals for the department. Works closely with Strategy and Business Development on growth initiatives for the service line.
  • Directs the development and monitoring of departmental budgets and all related expenditures staffing, space, equipment and other resources needed to ensure quality, safe, efficient and cost effective operations. This includes developing the department's operational and capital budgets, meeting financial and productivity targets.
  • Responsible for initiating and monitoring requirements of all outside contracts such as meals, transportation and grant funds. This includes tracking expenditures and service delivery for accurate reporting.
  • Establishes and maintains clear lines of communication among all staff and conducts regular staff meetings.
  • Develops and assures compliance with CommonSpirit Health policies and procedures.
  • Implements strategies to positively impact customer service scores such as participant satisfaction and employee experience.
  • Establishes and routinely monitors effectiveness of emergency procedures.
  • Ensures all department employees are appropriately trained, supported and evaluated. Initiates personnel actions in accordance with Human Resources policy and organizational philosophy. Monitors conditions of employment such as education compliance, licenses and certifications.
  • Ensures the maintenance of all licensing and certification compliance standards as required by Federal, State, County and other regulatory agencies. Leads the departmental ongoing announced/unannounced compliance activities and acts as a resource to surveyors. Maintains current knowledge of regulatory standards and interpretation and communicates new information to the appropriate personnel.
  • Ensures overall program philosophy is well defined with attention to person-centeredness, empowerment, independence and support. Ensures that ADHC is current on issues of aging, living with disabilities, cultural competency, caregiver support, community resources and effective service delivery.
  • Coordinates outreach activities to maintain a community presence and connection to referring agencies to ensure census targets are met or exceeded.
  • Must be an active and contributing member of the Woodland Memorial Hospital management team.
  • Identifies grant opportunities and actively participates in development and implementation of funding awards.
  • Ensures all department employees are well briefed on hospital activities, policy changes and new initiatives.
  • Actively participates as a leader in the older and disabled adult provider network in Yolo County as a member, at a minimum, of the Yolo County Adult and Aging Commission, Yolo Healthy Aging Alliance and Yolo County Health Council.
  • Promotes compassionate understanding of issues related to aging and the elderly.
  • Performs other related duties as assigned or requested in a professional manner.

Qualifications

Minimum Qualifications: 

Education and Experience: 

  • Minimum five (5) years of management or supervisory experience in community- based services with understanding of working with a clinical team.

  • Demonstrated skill in budget, program management, operations, staff supervision, community relations, marketing and excellent interpersonal skills.

  • Bachelor's Degree- Bachelor's degree in administration, nursing, social work, psychology, or recreational, occupational or physical therapy. 

  • Master's Degree in business administration, social work, public health, public administration or related field preferred. 

Special Skills: 

  • Knowledge of and/or ability to learn federal state and local regulations pertaining to the operation of adult day health care.
  • Strong computer skills capable of learning multiple new software programs/applications and navigating comfortably within Google Workspace.
