Responsibilities

Job Summary:

The Assistant Social Worker works under the supervision of the Medical Social Worker (MSW). Social worker provides psycho social assessments develops care plans and monitors treatment plans; assists with resolving problems that impede the participants rehabilitation and/or recovery process. The Assistant Social Worker makes referrals to appropriate community resources confers with other disciplines participates in internal meetings and completes paperwork in a timely manner. Responsible for program inquiry jobs intake coordination scheduling assessments and completing enrollment. Coordinates student intern and volunteers including recruitment training and ongoing supervision.

Responsibilities: 

  • Provide psycho-social participant assessments.
  • Develop collaboration with team, participant, family, and physician. Plan of care to address participant needs as related to diagnosis.
  • Provide appropriate intervention to include referrals to community agencies, assist patient with completion of forms, etc., provide counseling, advocacy role with outside resources, financial counseling, assist with appropriate living environment, facilitate group work.
  • Collaborate and educate team members to ensure appropriate psycho-social intervention/staff support to process referrals, i.e., CPS/APS/Ombudsman etc.
  • In collaboration with MSW assists with program marketing.
  • Submit timely and accurate documentation of intervention.
  • Assist in educating community regarding issues of aging, dementia, care-giving, etc.
  • Coordinate program intake process including initial screening, scheduling visit day, facilitating eligibility determination with health plans, scheduling assessment and finalizing enrollment.
  • Assists MSW recruiting and coordinating student interns and volunteers.
  • Provide information to participants, family and staff regarding eligibility determination for health plans, Veteran's Benefits, social security, in-home support services and other related agencies.

Qualifications

Minimum Qualifications: 

Education and Experience: 

  • Bachelor's degree from an accredited school in social services or related fields.
  • Demonstrated competence in needs of frail, chronically ill, elderly and disabled.
  • One year experience providing services to above population.

Licensure: 

  • Basic Life Support (BLS)

Skills: 

  • Ability to collaborate effectively with community agencies.
  • Demonstrate knowledge and skills relevant to the principles of growth and development over the life-span of the participant.
  • Perform psycho social assessments and formulate appropriate plans.
  • Assess and report incidences of suspected child abuse, dependent adult/elder abuse and domestic violence.
  • Communicate psycho social needs of the participant and family to the interdisciplinary team.
  • Maintain participants medical records. Maintain confidentiality.
  • Work with students and volunteers.
  • Facilitate support groups.
  • PHYSICAL FACTORS: Physically able to walk, stand, stoop and lift. Good manual dexterity. Visual and auditory acuity.

Knowledge: 

  • Knowledge of the principles and practices of clinical social work in the healthcare setting. Individual and group consultation process. Principles of grief and loss counseling. Crisis intervention techniques. Participants rights, ethical considerations and cultural differences. Community resources. Chemical dependency and mental disorders. Dementia and caregiver issues

Preferred: Bilingual Spanish
