AmeriMed provides home infusion therapy services to help patients safely transition care to their home, alleviating long and unnecessary stays in the hospital. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy

As the Care Transition Coach (CTC) makes essential contributions to the achievement of the Organization’s objectives as a field-based business development representative.
- The CTC is a hospital based resource for infusion therapy and enteral therapy services offered by the organization.
- This position educates the hospital staff about home infusion services, increasing awareness and helping identify when home infusion is an optimal solution and appropriate for individual patients.
- The CTC collaborates with referral sources in the process of providing coordinated quality home based services for patients and assists with all infusion therapy and enteral therapy referrals.
- Maintains knowledge necessary to serve as an educational resource for referral sources regarding product and payor coverage for infusion therapy, enteral therapy and durable medical equipment
- Develops and maintains relationships with referral sources including but not limited to Care Coordinators; Managed Care case managers; physician offices, etc. regarding Infusion services.
- Supports marketing efforts and meets or exceed established monthly admission target(s) as provided by their manager and determined by the branch monthly budget.
- Follows referral of a patient to the organization and assists discharge coordinators in coordinating quality home care services for patients in the following manner: Determines patient eligibility for Infusion services. Verifies insurance coverage. Effectively communicates with appropriate disciplines involved in the care of potential home Infusion patients, and provides information and recommendations to CommonSpirit Health at Home staff.
- Develops professional working relationships with health care providers and facilities, generating an open flow of information and support of Infusion goals; provides value added counsel, teaching and resourceful problem solving.
- Available to all discharge planners, physicians, other hospital personnel and patients to: ▪ Analyze eligibility for Infusion and provide general guidance in determining if a patient would benefit from Infusion
- Explain Medicare and Medicaid guidelines, insurance plan benefits, financing options, and organizational policies, etc.
- Observes and reports trends, changes, and identifies new opportunities for business.
- Participates in Performance Improvement (PI) Program Coordination; including assisting with the overall development and implementation of the PI plan, assisting in the identification of goals and related patient outcomes, participation and reporting of activities and outcomes, development and implementation of corrective plans.
Qualifications
- High school diploma or equivalent.
- Three years of experience in home infusion admissions.PREFERRED, Sales or marketing experience.
- Strong written and verbal communication skills. Ability to function well as part of team.
- Excellent organizational and time management skills. Excellent attention to detail.
- Multiple software programs including Windows-based programs.
- Medical Terminology. Customer service focused.
- Excellent training planning and polished presentation competencies.
- Excellent interpersonal, customer service and persuasiveness competencies; listens, gains respect, develops trust and acceptance of ideas; able to assertively build a network of referrals.
- Strong assessment critical thinking, judgment, creativity and decision making skills. Knowledge of home care regulations.
- Able to provide own transportation.
