The posted compensation range of $34.50 - $44.30 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Responsibilities
The Emergency Department Screening Program (EDSP) Coordinator will be responsible for the successful coordination of the HIV, hepatitis C and syphilis screening program, a population health improvement initiative managed by the Community Health Department. The coordinator shall be responsible for daily task supervision of each team member to ensure the goals, objectives and deliverables of the program are met. The coordinator shall provide training of new team members. The coordinator will organize provider and patient education, develop program dissemination materials, and coordinate system alignment work. He/she shall support the development and maintenance of program
materials, data tracking tools, and the program database. Responsibilities include and are not limited to: gathering and analyzing data from all sources to ensure data collection accuracy and timely reports, support the public dissemination of data (i.e. conferences, abstracts,manuscripts, etc.), work with patients to deliver person-centered care plans, follow-up, and resources.
The coordinator will collaborate with physicians, nurses, hospital case managers, laboratory staff, and other community agencies/organizations and healthcare provider offices to coordinate care and patient needs. The coordinator shall also provide technical support and assistance to CommonSpirit Health (CSH) Research Institute, CSH Population Health, and other CSH sites to inform program implementation and system alignment program efforts. The coordinator will provide support and guidance to the program’s patient navigators as needed. The coordinator will support the development and lead implementation of preventative strategies to reduce the positivity rates in the community.
Qualifications
Minimum:
- Minimum of one (1) year Community Outreach experience.
- Independent thinking, flexible and self-directed. Comfortable working in underserved communities. Excellent verbal and written communication skills required.
- Knowledge of community needs and existing health resources.
Preferred:
- Networking experience with community based organizations desired.
- Bachelor's Degree is preferred
- Bi-lingual in Spanish or one of the threshold languages of San Joaquin County preferred.
- Familiar with statistics, quality improvement methodology, and adult learning principles a plus.
