St. Josephs Behavioral Health Center is a licensed not-for-profit facility providing comprehensive behavioral health services for psychiatric and chemical dependency disorders. We offer a 35-bed inpatient hospital day treatment and outpatient services. Our specially trained staff of board certified psychiatrists physicians and licensed professionals are committed to providing compassionate quality care and creating an atmosphere that fosters communication trust and personal growth.


Responsibilities

Job Summary:

The Director of Care Coordination is responsible for providing leadership, program development and process improvement in the area of Clinical Social Work throughout the assigned Region. Works with regional leadership to create programs, processes and community alliances that will successfully support seamless patient transitions to post-acute care.

Responsibilities: 

  • Provides oversight in the development and standardization of policies and procedures for CSW in alignment with organizational strategic initiatives, staying focused on quality of patient experience and financial outcomes.
  • Develops and supports standardized orientation and training programs for Clinical Social Work The CSW training and expertise with the patient-in-their- environment and strength-based assessments and interventions is professionally focused to contribute to the Care Coordination model across the care continuum with an emphasis on transitions to community based care.
  • Develops and supports standardized orientation and training programs for Clinical Social Work The CSW training and expertise with the patient-in-their- environment and strength-based assessments and interventions is professionally focused to contribute to the Care Coordination model across the care continuum with an emphasis on transitions to community based care.
  • Provides liaison functions to community resources, creating strategic partnerships and alliances to benefit patient population.
  • Develops regional programs that support positive patient outcomes and maximum staff support such as Critical Incident Debriefing, Negative Outcome Management (managing patient and family in sentinel events/sensitive perceived care failure situations) in conjunction with Risk Management [i.e., Candor Program].

Qualifications

Experience:

  • Minimum of five (5) years experience in healthcare managerial positions.

Education:

  • Masters' degree or equivalent in Social Work or healthcare/business related field or a combination of education and experience.

License:

  • Current CA License for Social Work.
