Responsibilities

The Oncology Patient Navigator (OPN) is a member of the Cancer Care Navigation team that includes RN Navigators, Patient Navigators and Financial Navigators. The OPN provides support and coordination of care across the care continuum from screening through survivorship, serving as a central point of contact for patients and families. The OPN role includes guiding patients and their families through the complexities of the healthcare system, advocating, responding to questions, assisting in the coordination of clinical appointments and coordinating referrals. The OPN provides scheduling and care coordination, and enhances the patient-physician relationship and institutional loyalty.

  1. Bridge the gap between clinician and patient by providing individual support and assistance in navigating the healthcare system to provide continuity of care.
  2. Work with RN Navigator disease team to provide non-clinical support regarding scheduling, follow-up and record retrieval for timely appointments.
  3. Call new patients within 24 hours of visit to the cancer program to check in regarding follow-up appointments and needs.
  4. Work with the multidisciplinary team (RN Navigators, Social Workers, Dieticians, Genetic Counselors, Providers, Imaging, Pathology, treatment centers) to coordinate and expedite care.
  5. Partner with the Transition Center to assist patients in transitioning from inpatient to outpatient areas to avoid interruption in services and care.
  6. Connect patients to services for support (Social Work, Financial Counseling, Nutritional services, etc.)
  7. Track all patient contacts, follow-ups, barriers to care and outcomes per institutional and program guidelines.
  8. Ensure exceptional patient experience through the timely coordination of care and by keeping patients informed of their care plan and process.
  9. Participate in interdepartmental activities to ensure the delivery of high quality, cost effective care.
  10. Participate in disease specific cancer conferences and interdisciplinary meetings.

Qualifications

Required:
- Associate's degree or equivalent experience
- 2 years' experience working in a healthcare office setting or community outreach/education
- Minimal business expertise
- Significant problem-solving skills, particularly with respect to addressing barriers to care and navigating patients through their care progression.
- Exceptional interpersonal skills as this role supports a population of patients across many clinical specialties and stages of care.

Preferred:
- Bachelor's degree or equivalent experience plus 2 years' experience working in a healthcare office setting or community outreach/education
- Clinical education and experience (i.e. MA, CNA, LPN, etc.)
