Responsibilities

Responsibilities:

Bring your passion for oncology and patient care to the outpatient infusion center at Dignity Health Cancer Institute at St. Joseph’s Hospital and Medical Center. As a staff pharmacist you will be part of a growing program on the forefront of cancer treatment. We are looking for someone that is a great communicator, detail oriented, self-motivated, and able to problem solve through critical thinking. 

Staff pharmacists work collaboratively with physicians, nurse practitioners, nurses and other members of the care team. Cancer Institute pharmacists are viewed as medication experts who focus their time on supporting nursing staff in the infusion suite, offering patient education, and participating in clinical decision making. They review and create care guidelines, and utilize Cerner to assist with appropriate drug therapy selection, management, and monitoring.

Principal Duties and Accountabilities:

  • Provides pharmacy services to the outpatient infusion service line to reduce harm to patients and improve patient outcomes. Follows all regulatory, accreditation and hospital standards of care, including USP chapters and department policies.
  • Reviews and assesses patient medication profiles to identify, prevent and resolve potential and actual drug related problems.
  • Utilizes clinical pharmacy skills in assessing and formulating recommendations to physicians ensuring positive patient outcomes, safety, quality and cost effective drug therapy.
  • Provides accurate, appropriate and timely information to physicians, nurses, patients and others regarding medication therapy, drug product guidelines, law and regulations, medication formulary and other topics related to the medication use process.
  • May be cross-trained to work in the Barrow Neuroplex infusion pharmacy.
  • Coordinates medication preparation and distribution work of the pharmacy technicians and ensures accuracy prior to dispensing medications for distribution to the patient care areas.
  • Maintains established procedures concerning quality assurance, security of controlled substances, and proper disposal of hazardous waste

Qualifications

REQUIREMENTS:

Bachelor’s Degree or Doctorate in Pharmacy from an accredited college of pharmacy

One year of work experience in hospital or outpatient oncology practice

PREFERRED QUALIFICATIONS:

PGY-1 Pharmacy Residency or 3 years of hospital pharmacy or home infusion experience

Oncology Board Certification (BCOP)
