Responsibilities

Provides psychosocial assessment discharge planning/coordination community referrals crisis intervention and supportive counseling. Develops and disseminates educational material to physicians nurses and staff regarding community resources healthcare issues insurance legislation entitlement programs etc. Facilitates education programs for patients and families.

  • Develops and disseminates educational material to physicians, nurses, and staff regarding community resources, healthcare issues, insurance legislation, entitlement programs, and patient and family psychosocial issues.
  • Facilitates meetings and conferences between providers, patients, and their families.
  • Negotiates patient coverage issues with health insurance companies.
  • Presents and facilitates education programs for patients and families.
  • Facilitates patient support and education groups.
  • Collaborates with community resources in securing patient services.
  • Provides direct patient and family counseling.
  • Provides professional expertise to patients in applying for FMLA, SSDI, SSI, food stamps, AHCCCS, COBRA insurance.
  • Assists families in securing advance directives.
  • Assists patient and family members in decisions regarding treatment, home care issues, aggressive treatment vs.palliative care.
  • Provides conflict resolution between family members and patient care team.
  • Arranges for crisis intervention for Barrow Neurology Clinic patients.
  • Presents and facilitates education programs for BNC providers and staff.
  • Provides liaison services between BNC providers and community providers.
  • Represents patients in advocating their rights for workplace accommodations under the ADA (Americans with Disabilities Act).
  • Educates providers and patients on workplace rights and accommodations.
  • Provides medical updates to case managers for patients on short and long term disability.
  • Assists patients in writing appeals for benefit programs.

Qualifications

MINIMUM

Master's Degree in Social Work

Five (5) years of social work experience required

Basic computer skills 

Licensed Master Social Worker required.
