The House Supervisor/Clinical Command Center Nurse is responsible to provide and coordinate care to individuals and families using a holistic approach in our onsite clinical command center. The nursing training and experience is used as a structured mechanism for critical thinking and problem solving in providing effective and appropriate care within a safe and confidential environment. The incumbent is responsible for collaborating with members of the divisional and national healthcare teams to provide appropriate care, education, and support to patients, family, and providers. The incumbent will be able to effectively and efficiently speak with patients to facilitate progression of care, including patient education, assessment of clinical status and providing interventions that are patient specific partnered with the patient/family. The incumbent directly supports the organization’s efforts to enhance the patient experience, promote patient safety and improve risk reductions.
As a Clinical Command Center RN, the Nurse will work with multiple staff, clinicians and leaders to support all aspects and programs in the clinical command center; including but not limited to Virtual Nursing, Connection Specialists, Virtual Companions, Cardio-Pulmonary Technicians, Patient Placement and Transfer Center. Floating to the area of need to support Clinical Command Center operations.

- Will function as a lead and resource for all Clinical Command Center functions and to float to the role needed to support patient care, throughput, and transitions.
- Will work well in a matrixed reporting structure
- Will identify areas of opportunity and work with leadership to develop and implement solutions
- Will observe processes and utilize the PDCA Cycle, (Plan, Do, Check, Adjust) to make rapid improvement to workflow, and to effectively communicate these changes to the teams.
- Assists with departmental orientation including, teaching and mentoring new and incumbent staff to ensure understanding of policies, procedures and workflow.
- Collaborates and communicates effectively virtually with physicians, nurses and other health care team members in the provision of clinical care across the continuum
- Assesses and reassesses virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history and monitoring and evaluating diagnostic studies as medically indicated.
- Interprets available data in order to establish a primary plan of care and intervenes as required. Participates through an interdisciplinary team approach and through multi-dimensional patient care conferences, ongoing patient assessments, and re-evaluation of patient problems or needs to develop a plan of care.
- Prioritizes clinical information with critical thinking to escalate patient instability with care plan and medical regimen
- Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decision-making and provides follow-up to ensure that patient problems are resolved.
- Provides direct observation via virtual technology, proactive surveillance of adverse physiological trends, and assurance of appropriate prophylactic strategies, medical chart review, and appropriate reaction to appropriate alerts.
- Prioritizes and manages multiple electronic devices and software applications including but not limited to EMR, Call Management Systems, Patient monitoring tools, Google workspace, and online schedules.
- Coaches and mentors other clinical team members
- Virtually educates the patient and families on disease processes and procedures.
- Works to exceed national benchmarks for quality care delivery, cost containment and length of stay indicators.
- Maintains patient privacy and confidentiality
- Responsible for staying current in department policies and workflows, in-service programs, continuing education programs, and fulfills yearly requirements as stated by policy
- Participates in achieving nursing division goals and supports a shared leadership structure.
- Assures appropriate level of understanding, awareness and compliance with all applicable with the Joint Commission (TJC), federal, state and agency laws, regulations, guidelines and professional standards.
- Through a defined reporting process, reports clinical, administrative, or technical problems that result in medical errors, unsafe care, or sub-optimal communication between appropriate health care professionals.
- Demonstrates work practices consistent with CommonSpirit Health and department-specific safety, security and infection control policies.
- As a system employee, the incumbent understands the market, division, local culture, and relationships.
- Collaborates with internal and external patients/clients within and across systems to ensure appropriate care transitions.

Qualifications
- RN required
- Minimum of 5+ years of recent experience as a licensed nurse and knowledge of care transitions.
- Knowledge and ability to facilitate patient care across the continuum
- Experience and knowledge in the use of Electronic Medical Records (EMR)
- Bachelors of Science in Nursing preferred
- Cardiac monitoring experience preferred
- Current license Registered Nurse
- Nurse Licensure Compact (NLC) or multi-state licensure
- Valid Basic Life Support (BLS) certification required
- Knowledge of Advanced Life Support (ALS) (preferred)
- Ability to use multiple technologies; audio/visual
- Ability to build alliances in a highly-matrixed environment. Demonstration of consistent integrity and ability to meet commitments

- Communicating ideas and information clearly, concisely and effectively
