Provides direct social work services including assessment, treatment planning, intervention, counseling, and recommendations to individual patients and their families/significant others and/or groups of patients at the Henry Lynch Cancer Center at CUMC-Bergan Mercy.

The social worker is knowledgeable in areas such as active listening, social perceptiveness, critical thinking, and coordination.


Responsibilities
  • Conduct and document psychosocial assessment of patients 
  • Share responsibility in identifying appropriate decision makers if the patient is without capacity, educating all patient’s rights and encouraging participation in decision-making and care
  • Provide liaison services and referrals to hospital resources and community agencies 
  • Identifies mental health needs of patients and provides counselling and referral as indicated 
  • Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community.  Informs other members of the health care team about the factors that may affect the patient’s health and well-being
  • Serves as a resource to patient, family, and healthcare team regarding emotional and behavioral issues and refers to community services available to adolescents, adults, and elders
  • Remains current of Medicare and Medicaid laws and guidelines, Medicare conditions of participation, managed care contracts, and other legal and regulatory requirements as the relate to healthcare social work services. Assures patients are informed of rights and responsibilities as participants in these programs

Qualifications

Required Education for Staff Job Levels

Master’s Degree from a Council of Social Work Education accredited School of Social Work

Required Licensure and Certifications

State of Nebraska certification/licensure in Social Work required
