Program Coordinator - Community & School Health Initiative

Northridge Hospital Medical Center

Northridge Hospital Medical Center is seeking a passionate and dedicated Program Coordinator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role for outreach and engagement to provide direct referrals to services for people who were the most affected by the pandemic. The goal is to further reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education. 


Responsibilities
  • Supervise Community Health Navigators (CHN's) and Data Coordinator that they deliver program services.
  • Manage program budget expenses, and ensure compliance with grant requirements.
  • Coordinate with community partners and stakeholders to ensure program effectiveness and reach.
  • Identify and engage with community members, particularly those from underserved populations.
  • Build relationships with community leaders, organizations, and institutions.
  • Provide ongoing support and mentorship to staff, ensuring their skills and knowledge are up-to-date.
  • Analyze program data to track progress, measure impact, and inform program improvement.
  • Prepare regular reports for funders and stakeholders, highlighting program achievements and challenges.
  • Other duties as assigned

Qualifications
  • BS/BA degree required  in Health Administration, Public Health or Public Policy or similar field required, 2 years of experience as a Community Health Worker or System Navigator, or 5 years’ of experience as a Community Health Worker, or System Navigator, or in an equivalent role, with at least 2 years at the level of Senior Community Health Worker or Lead System Navigator in a public or private community based organization, clinic, hospital, public health organization, or health system.
  • Knowledge of budgeting or accounting. 
  • Knowledge of local community agencies and resources. 
  • Ability to organize projects and establish priorities.
  • Creative and analytical skills. 
  • Grant writing experience preferred.
  • Working knowledge of Microsoft 365 and Google Workspace products Zoom and Canva. 
  • Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
