AmeriMed Infusion Pharmcy in Louisville, KY, Now Hiring Full-time Care Transition Nurse

AmeriMed provides home infusion therapy services to help patients safely transition care to their home, alleviating long and unnecessary stays in the hospital. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy


Responsibilities

Now Hiring Full-time Care Transition Nurse for the Louisville, KY location

The Care Transition Nurse (CTN) makes essential contributions to the achievement of the organization’s objectives as a field-based business development representative, consistently meeting the volume expectations for referrals and admissions.

  • Follows referral of a patient to the organization and assists discharge coordinators in coordinating quality home care services for patients in the following manner:
  • Determines patient eligibility for Infusion services.
  • Effectively communicates with appropriate disciplines involved in the care of potential home Infusion patients, and provides information and recommendations to CommonSpirit Health at Home staff.
  • Develops professional working relationships with health care providers and facilities, generating an open flow of information and support of Infusion goals; provides value added counsel, teaching and resourceful problem solving.
  • Available to all discharge planners, physicians, other hospital personnel and patients to:
  • Analyze eligibility for Infusion and provide general guidance in determining if a patient would benefit from Infusion services.
  • Explain Medicare and Medicaid guidelines, insurance plan benefits, financing options, and organizational policies, etc.
  • Upon patient choice of the organization, communicates with attending physicians to obtain verbal or written orders for home care services.
  • Assists in ordering equipment as required to provide proper home Infusion for the patient referred to the organization.
  • Upon patient choice of the organization determines individual patient needs for services, community referrals, training and provides patient instructions on equipment and procedures, etc.
  • When requested, participates in educating and in-servicing new providers and personnel on home Infusion and the importance of the communication of this information to their clients.
  • Provides general Infusion education to patients, caregivers and others as requested by referral sources.
  • Meets with the Director, Operations and other organizational employees to ensure continuity of care
    for the patient.
  • Functions in clinical areas which may include neonates, pediatrics, adolescents, adults, and geriatrics. May interact directly with persons in the age categories of neonate, infant, child, adolescent, adult and geriatric.

Qualifications

  • Graduate of an accredited school of nursing.
  • Experience as a Care Transition Nurse or similar role, sales or marketing.
  • Hospital and/or homecare nursing experience.
  • Current RN, or LPN, license to practice in the state serviced.
  • Obtain and maintain current CPR certification; able to perform CPR.
  • Compliant with appropriate state certification/registration and organizational requirements as necessary.
  • Two years of appropriate nursing experience.
  • Knowledge of home care regulations required.
  • Strong assessment, critical thinking, judgment, creativity and decision making skills.
  • Excellent communication skills; both verbal and written.
