Dignity Health @ Mercy Hospital of Folsom is the community hospital serving the Sierra foothills communities of Folsom El Dorado Hills Granite Bay Cameron Park Shingle Springs and Rescue. With 106 licensed acute-care beds our accredited non-profit hospital has been providing exceptional care for more than 35 years. Mercy Hospital of Folsom provides surgical services including minimally-invasive general urological and gynecological surgeries. We also provide comprehensive and award-winning orthopedic services which include surgery inpatient and outpatient rehabilitation and specialty orthopedic care. As a certified Joint Commission Stroke Center Mercy Folsom provides exceptional care to our patients in our Medical Surgical and Intensive Care units as well as our 25-bed Emergency Department. Our Family Birth Center features private and contemporary birthing suites where labor delivery and recovery occur in one room as well as award-winning lactation training and support. Mercy Folsom received Among America’s 250 Best Hospitals recognition and is a Primary Stroke Center.


Responsibilities

Position Summary

The Wound Care Coordinator is responsible for the development coordination and monitoring of the hospitals Wound Care Program. The Wound Care Coordinator establishes standards of care and clinical competencies to meet patient needs and training to ensure clinical expertise. The Wound Care Coordinator works collaboratively with the medical staff and provides education to patients caregivers and staff on wound ostomy and continence care. This position requires the full understanding and active participation in fulfilling the Mission of Mercy Hospital of Folsom. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Mercy Hospital of Folsom’s strategic plan and the goals and direction the quality improvement / process improvement activities.

Position Scope

  • Develops and implements the hospitals Wound Care Program to include patient care protocols documentation tools wound care formulary and WOCN referral criteria.
  • Establishes standards of care competencies policies and procedures in quality cost-efficient and effective wound care for all clinical staff.
  • Makes recommendations to the medical staff for changes to wound care orders and provides evidence-based research support as needed
  • Provides instruction and guidance both formally and informally to staff and assists in identifying staff learning needs related to wound ostomy and continence as appropriate.
  • Assures that new equipment and procedures are inserviced to all staff members.
  • Performs weekly skin/wound rounds on selected units/patients
  • Provides wound or ostomy consultation at the request of any multidisciplinary team member.
  • Monitors/approves rental equipment in relation to pressure ulcer prevention and treatment and wound management.
  • Collaborates with Case Management for patient referral as needed at discharge.

Qualifications

Minimum Required

  • Current CA RN licensure and AHA BLS
  • Bachelor’s Degree in Nursing (BSN)
  • Three (3) years experience as a Registered Nurse in an acute setting in the last 5 years.
  • RNs with subacute experience may be considered based on the complexityof wound care performed within the subacute setting
  • At time of hire a Wound Care Certification (WCC) or Certified Wound Ostomy Nurse (WOCN)
  • Within 24 months of hire the following must be obtained:
    • Wound Ostomy and Continence Certification (WOCN) or Ostomy Management Specialist Certification (OMS) with Certified Wound Care Nurse Certification (WCC)
