Responsibilities
  • Clinical Practice: utilizes appropriate resources to gather data and assess patient care needs; Makes ongoing assessments of the following domain: physio-biological, cognitive/emotional, spiritual and social, managing life with advancing disease, dying and end of life care; Identifies problems that could lead to an emergency/crisis situation and take appropriate action; Identifies the need for and coordinates consultants; Communicates assessment data to appropriate persons; Assesses learning needs of patient/significant others; Assesses discharge needs of patient/significant others; Documents assessment data according to unit standards; Plans and manages daily care for group of patients; Incorporates medical and nursing priorities into patient care; Formulated individualized nursing care plans based on data based expected outcomes; Plans interventions appropriate to patients medical and nursing diagnoses, age, abilities and resources; Establishes and implements learning plan for patient/significant others; Establishes and implements discharge plan for patient/significant others; Documents planning process according to organizational and until standards; Manages physiological state including: pain, neurologic care, cardiac care, respiratory care, hemodynamic regulation, invasive and noninvasive monitoring and fluid and electrolyte balance; Manages mental, emotional and cognitive state; Manages nutritional state; Monitors infection control; Provides teaching to patients/families related to patients diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; Documents each elements of care per organizational and unit standards; Assesses the quality of care on a ongoing basis; Determine appropriateness of medical and nursing treatments; Evaluates patient response to medications, treatments; and Documents professional assessment of each aspect of evaluation process per organizational and unit standards.
  • Collaboration, delegation, supervision: Forms partnerships with all members of the interdisciplinary team; Communications with coworkers and other departments in a professional manner; Shares appropriate information with all members of the health care team, including assistive/support personnel, in a manner that is appropriate and timely for the patient's condition; Participates in team rounding; Exercises reasonable judgment in delegating activities in terms of potential harm, complexity of care needed, predictability of the outcome and competency and training of the support personnel; provide clear direction/explanation of the task; Maintains the level of supervision that is appropriate to the delegated task, assesses the performance of support personnel in the performance of the these tasks, and intervenes as necessary giving prompt and specific feedback; Provides periodic inspection of task accomplishment; and Monitor patient and family satisfaction with the delegated care.
  • Profession and Leadership responsibilities: maintains high ethical standards in practice; Advocates for the patient and family and protects patient's autonomy, dignity and decision making rights; Maintains confidentiality of all patient information and holds others accountable for maintaining confidentiality; Delivers care in a non-judgemental and nondiscriminatory manner; Demonstrated sensitivity to cultural diversities of patients and coworkers; identifies, documents and report incidents per organization standards; Supports mission of VMMC; Supports philosophy of patient centered care and assists support staff to do so; Supports belief that anchor the documentation system and documents according to established standards; Independently meet medical center requirement updates on health and safety, blood glucose and CPR, renews RN license and presents to manager or staffing office; Maintains fiscal responsibility and incorporates the value equation into daily practice; Extend role beyond walls of the institution through awareness of current legislation related to nursing practice and health care policy; Involves self in community service projects; Facilitates resources for patients across the continuum care; Maintain satisfactory attendance record; Maintains professional appearance; Attends and participates at staff meetings; Participates in interdepartmental and work unit committees/task forces to improve practice, enhance communications and improve work flow; Give input into standards of care, nursing procedures and clinical pathways; Seeks productive work during quiet times; Accepts change as part of systems improvement and acts as a positive change agent; Contributes to a positive, collaborative working environment; participates in orientation and mentoring of new employees in a formal and informal manner; Contributes to the professional growth of coworkers by sharing knowledge and experience; Offers constructive feedback to coworkers; Accepts constructive feedback from coworkers and implements change in practice when needed; Participates in evaluation process; Contributes to a positive work environment, including resolution of conflicts with colleagues and customers in a positive manner.
  • Education and Research: Maintain clinical and professional expertise through continuing education; Update knowledge base by attending inservices on new equipment and procedures; Participates in CQI activities; Uses results of CQI initiates to change own practice; Supports and participates in research activities as appropriate and incorporates research findings into clinical practice.
  • Develops a presence in the community by maintaining relationships with various agencies and service providers to effectively market the programs.

Qualifications
  • A current and valid Washington State Registered Nurse license is required for this position. CPR certification is required every two years or as specified by the work. Other certifications may be required for specialty areas. This job also requires excellent assessment/triage knowledge and skills; excellent communication skills; a thorough working knowledge of clinical practices and procedures; an understanding of chemical dependency; excellent communication skills; an understanding of HIV/AIDS disease progression; strong organizational skills; and excellent counseling/psycho-social skills.
