Responsibilities
This position is responsible for providing and facilitating activities and services regarding care for patients with specific diagnoses related to subspecialties. This involves screening the inpatient, emergency department, and other specialty referral lists to identify the applicable patients who will benefit from enrollment in these sub-specialty clinic(s) or services. Schedule tests and coordinate referrals for complex patients with associated diagnoses treated in the sub-specialty clinic(s). Responsible for overseeing activities in the clinic, coordinating patient care and evaluation, providing education, design and aligning resources to support program initiatives, program development, develop and maintain relationships with internal and external referring physicians, and interact with the multidisciplinary team, leadership and industry and development of a program database.
Essential Key Job Responsibilities:
Patient Referral:
- Facilitates referrals to the sub-specialty clinic. Coordinates appropriate testing and collates results for review. Utilizes appropriate software for coordination of images/studies.
Patient Care Coordination:
- Serves as a clinical resource for identification, tracking and treatment alternatives for patients with applicable diagnoses being treated in the sub-specialty clinic(s).
- Serve as the primary point of contact for patients with applicable sub-specialty diagnoses, facilitating communication between patients, families, and healthcare providers.
- Develop and implement individualized care plans in collaboration with sub-specialists, primary care physicians, and other healthcare team members.
- Monitor patient progress longitudinally, addressing any issues or complications that arise and adjusting care plans as needed.
- Creates a tracking system/database to follow patients. Facilitates and coordinates sub-specialty services.
Education and Support:
- Provide education to RNs and MAs, patients and their families about sub-specialty diagnoses, including medication adherence, lifestyle modifications, and symptom monitoring.
- Offer emotional support and counseling to help patients cope with the challenges of living with these diagnoses.
- Coordinate and conduct educational workshops or support groups for patients and families.
Case Management:
- Assess patient needs and coordinate necessary services such as home health care, physical therapy, and dietary counseling.
- Assist with scheduling and facilitating follow-up appointments, tests, and procedures.
- Ensure timely and accurate documentation of patient interactions and care plans.
Quality Improvement:
- Participate in quality improvement initiatives aimed at enhancing care and patient outcomes.
- Collect and analyze data related to patient outcomes, hospital readmissions, and adherence to treatment protocols.
- Collaborate with healthcare teams to develop and implement best practices for management of these diagnoses.
Resource Management:
- Identify and connect patients with community resources and support services, including financial assistance programs and transportation services.
- Assist with navigating insurance and healthcare systems to ensure access to necessary treatments and medications.
- Maintains sub-specialty database/registry for clinic patients, device patients and related referrals, consistent in collaboration with the sub-specialty director; provides outcome reports from these registries to the administrative teams and providers.
- Other duties as assigned by management.
Qualifications
Minimum Qualifications:
- Current/Valid RN license from the state Licensing board required.
- Basic Life Support (BLS) Certification required.
- Three to five years of Cardiac, Thoracic, or Critical Care Nursing experience required.
