As the Clinical Services Consultant, you hold the key to the success of multiple Home Health and/or Hospice operations, leading, coordinating, and facilitating patient care with unwavering dedication and integrity.
Responsibilities
The Clinical Services Consultant is a high functioning RN leader, available to support agency locations for interim needs in managing all or portions of the Director, Clinical Services/Area Director, Clinical Service role, via remote support and deployment to the agency. When not actively deployed or assigned to a specific location, the Clinical Services Consultant will be assigned to other clinical leadership projects, assignments, or tasks for which they are qualified.
The Director, Clinical Services (DCS)/Area Director, Clinical Services (ADCS) is responsible for the planning, direction, coordination, supervision, evaluation and provision of all clinical services provided by the assigned facility location. Oversees the coordination, continuity, and quality of clinical care, as well as the management of patient services and monitors, evaluates, and takes action on clinical measures of performance. Participates in the recruitment of employees, and provides the supervision necessary to facilitate the provision of quality care and patient satisfaction; and leads the education, training, and development of associates.
General Responsibilities
- Functions as an effective team member and contributes to the development and achievement of location(s) and organizational goals and objectives
- Ensures the effective delivery of clinical services to patients in accordance with company clinical, strategic, quality improvement and productivity standards.
- Oversees clinical operations and assures compliance and competency to home care/hospice policy, accrediting body’s standards and Federal, State, and rules and regulations.
- Ensure the accuracy, completeness and timeliness of clinical records and regulatory, operational and financial compliance.
- Lead the efforts for the location(s) to assure excellent operational, clinical and patient experience outcomes, driving improvements in areas as needed.
- Responsible to lead all State, Federal, or accrediting body surveys in location(s).
- Provides support to the Market Director, Operations.
- Manages interim direct reports.
Interim Clinical Services
- Responsible for directing the accurate, complete and timely provision of quality patient care and services in compliance with regulatory, operational and financial requirements.
- Assures the accuracy, completeness and timeliness of clinical records in compliance with regulatory, operational and financial requirements.
- Responsible for assuring compliance with all state, federal, and ACHC regulations and standards
- Responsible for ensuring that all HCHB clinical workflow for DCS as well as MCSs is completed and current by the end of each business day.
- Responsible for monitoring and evaluating clinical measures of performance including, but not limited to productivity standards, documentation completion standards, and management of patient care. Develops and follows through with action plans based upon measurement results
- Responsible for the ongoing development of clinical associate knowledge, understanding, and appropriate completion of OASIS, completion of initial and on-going physician orders, and all other clinical documentation tools.
Interim Quality
- Responsible for the location’s quality improvement and clinical excellence program.
- Assures effective and efficient use of all chronic care management programs in the location(s).
- Assures appropriate service utilization and appropriate OASIS scoring
- Reviews all occurrence and complaint reports and related follow up needed.
- Ensures Quality Assessment Performance Improvement (QAPI) program coordination; including collection and analysis of data measures identified by leadership and the governing body, agency QAPI Committee oversight, implementation of performance improvement activities, QAPI education to inform associates of improvement activities, ongoing evaluation of the effectiveness of improvement activities, documentation of QAPI activities and communication with agency and regional leadership on the program results, activities and effectiveness.

Qualifications
- Current Registered Nurse (RN) in good standing to practice in the state(s) of service.
- Must be eligible for licensure in all states CommonSpirit Health at Home operates and must apply for licensure in all applicable states within 60 days of hire.
- Minimum of two (2) years clinical home care experience and minimum of five (5) years of leadership experience are required.
- Must obtain and maintain current CPR or BLS Certification.
- Demonstrated ability to effectively lead, train, develop, establish and maintain behavior and performance expectations, evaluate and manage employee performance.
- Ability to travel. Overnight stays may be necessary.
